The Mind–Metabolism Connection: How Metabolic Health Affects Brain Health

Brain health is not only about memory. It also involves energy production, mood, sleep, attention and the brain’s ability to adapt and repair itself.

The brain makes up only about 2% of total body weight, yet it uses around 20% of the body’s energy at rest. To meet this demand, the brain needs a continuous supply of fuel.

Glucose is the brain’s primary fuel under normal conditions. However, during periods of fasting or carbohydrate restriction, the brain can also use ketones as an alternative energy source.

This ability to adapt between different fuel sources is sometimes described as metabolic flexibility.

When metabolic health is impaired by factors such as insulin resistance, poor diet, chronic inflammation and disrupted blood sugar regulation, the brain may also be affected.

The term “Type 3 diabetes” is sometimes used informally to describe the idea that brain insulin resistance may contribute to cognitive decline and Alzheimer’s disease. However, it is important to clarify that Type 3 diabetes is not an officially recognised medical diagnosis.

Research has suggested that insulin signalling plays an important role in the brain. Insulin is involved not only in glucose metabolism but also in processes related to learning, memory and neuronal function.

When insulin signalling becomes impaired, researchers have proposed that this may contribute to:

● Reduced glucose utilisation by brain cells

● Increased oxidative stress

● Neuroinflammation

● Impaired synaptic function

● Changes associated with cognitive decline

However, the relationship between brain insulin resistance and Alzheimer’s disease is complex and still under active research. It is therefore more accurate to describe this as a research concept or hypothesis, rather than saying that Alzheimer’s disease is simply “Type 3 diabetes.”

Changes in brain and metabolic health can sometimes appear gradually. These may include:

● Brain fog: Difficulty concentrating or thinking clearly

● Memory difficulties: Frequently forgetting recent information

● Mood changes: Increased irritability, anxiety or low mood

● Poor sleep: Difficulty maintaining restorative sleep

● Reduced mental energy: Feeling mentally exhausted or less focused

These symptoms are not specific to brain insulin resistance and can have many causes, so they should not be used as a diagnosis on their own.

How Gut Health, Inflammation and the Brain Are Connected

The gut and brain communicate continuously through the gut–brain axis, involving the nervous system, immune system, hormones and microbial metabolites.

A diet high in ultra-processed foods and added sugars, combined with low fibre intake and other lifestyle factors, may negatively affect the gut microbiome in some individuals. Changes in the gut microbiome have been associated with inflammation and metabolic disturbances.

Research is also investigating how gut-derived inflammatory signals and microbial metabolites may influence the blood–brain barrier and brain function.

This does not mean that every gut imbalance directly causes brain disease. Instead, it highlights an important area of research into how diet, gut health, inflammation and brain health may interact.

When Metabolic Problems Start Affecting the Whole Body

Poor metabolic health can have effects beyond weight and blood sugar.

Insulin resistance, chronic inflammation, high blood pressure and abnormal cholesterol levels can all contribute to cardiovascular and cerebrovascular risk.

Over time, these factors may affect:

Blood vessels: Increasing the risk of vascular disease and stroke

Brain health: Potentially affecting cognitive function

Hormonal regulation: Influencing appetite and energy balance

Metabolic health: Increasing the risk of type 2 diabetes and obesity

The important message is that brain health and metabolic health are interconnected.

● Eat a diet rich in vegetables, fruits, legumes and whole grains

● Include adequate protein and healthy fats

● Prioritise fibre and diverse plant foods for gut health

● Choose minimally processed foods more often

● Maintain healthy blood pressure, blood sugar and cholesterol levels

● Stay physically active

● Prioritise quality sleep

● Avoid smoking and limit alcohol

● Manage chronic stress

Brain health does not exist in isolation. The same habits that support healthy blood sugar, blood vessels and metabolism may also help protect long-term brain health. Your gut and brain are not separate systems. They constantly communicate and your diet can influence both.
